Supporting Medication Reviews for Your Loved One

Medications play an important role in health, particularly for older people living in residential aged care. Over time, medication regimens can become complex and may require review to ensure they remain safe, appropriate, and aligned with current care goals.

Ezymed supports families, carers, and care teams by providing clear information about medication reviews and ensuring consent processes are transparent and respectful.

What Is a Residential Medication Management Review (RMMR)?

An RMMR is a comprehensive review of medications for residents living in aged care facilities. It is conducted by an accredited pharmacist and focuses on:
  • ensuring medicines remain appropriate for the resident’s current health status
  • identifying potential side effects, interactions, or unnecessary medicines
  • supporting safer medication use and quality of life
RMMRs are requested by the resident’s GP and form part of a collaborative care process.

Family Consent & Involvement

Families and substitute decision-makers play an important role in understanding and supporting medication reviews.

Where required, Ezymed facilitates a structured consent and information process to ensure families are:
• informed about the purpose of the RMMR
• aware of what the review involves
• given the opportunity to provide consent or ask questions
